I acknowledge and understand that Old Blue Inc is a nonprofit organization offering financial assistance to pet owners for the costs of spay/neutering their pets at partnering veterinary offices.
By accepting this assistance, I hereby agree to the following terms and conditions:
1. I understand that Old Blue Inc is not responsible for performing the spay/neuter procedures. I acknowledge that these procedures, like any other surgery, carry potential risks including, but not limited to, reactions to anesthesia and surgical complications.
2. I accept that any unforeseen complications or actions that may arise before, during or after the procedure are not the responsibility of Old Blue Inc.
3. I understand that in an emergency during the procedure, the attending veterinary professionals will dictate the course of action. Old Blue Inc does not have any control over medical decisions.
4. I understand that I am responsible for disclosing all relevant health information about my pet, including current and past health conditions, to the veterinary professional performing the spay/neuter procedure.
I hereby release and discharge Old Blue Inc, its agents, volunteers, and employees from any and all claims, causes of action, demands, liability, costs or expenses, legal or otherwise, which may arise as a result of any injury, loss or damage related to the services financially assisted by or coordinated through Old Blue Inc including but not limited to, the spay/neuter procedure.
I certify that I have read and understand this liability waiver and I accept its terms and conditions.
